The page holds, in this order: your home average for the period against the goal, the trend since the last visit, the highest reading and when it happened, how many readings over how many days (so the average is trustworthy), the one change you tested and what the number did in response, your medications as actually taken (the dose, when, what you miss in a typical week, and what you feel), and your three questions for today.

That medication line deserves a sentence, because it is the one clinicians almost never get honestly: not the prescription, but what you actually take, what you skip, and the side effect you have been quietly tolerating. Written on a page, it stops being a confession and becomes data, and it is often the line that changes the plan. The printable closes with a "leave the visit with" box you fill in together: the target, the plan change agreed, and the recheck date.
